Please note that this is a preliminary application. Upon receipt and a successful phone interview, we will schedule a face-to-face interview.

The completed application can be either printed, faxed to 858-369-5951 or attached and emailed to info@wholelifehomecare.com.

Application Instructions

1. Click on the 'Download Application (PDF)' button above

2. Either choose to save the file to your computer or it will open automatically in your browser

(If you do not have a PDF reader [like Adobe Acrobat] installed on your system, you will not be able to fill out the application. You can download Foxit PDF Reader, a compact PDF reader)

3. Fill out the application by typing in the appropriate forms. Move your mouse cursor over the empty lines and boxes and the cursor will change from an arrow to a text input. Click and begin typing.

4. When you are finished filling out the application, or to save your progress and continue at a later point, locate the 'Save' button (it looks like a floppy disk) and save your progress. Or click on File, then click Save

5. To email the finished application, open your email client (Gmail, AOL, etc) and compose a message with info@wholelifehomecare.com in the 'To' field. In the Subject field, please type your full name and write 'Application'. Attach the completed PDF (it should be called application.pdf) to the email.

6. To fax the application, look for the 'Print' button on the PDF viewer (it should look like a grey printer). When the application has finished printing, send it as a fax to 858-369-5951
